ENTRY FORM FOR 2010 NATIONAL VETERANS DAY PARADE Mail form to Parade Chairman, PO Box 642, Birmingham, AL 35201, no later than October 30. The Parade Order will be mailed the first of November. 1. Name of Organization: __________________________________________________ 2. Type of Organization: Veteran Military JROTC Band Civic (please circle appropriate category) 3. School Classification: Elementary School Middle School High School College Military (please circle appropriate category) 4. Entry Information: This is the information the parade announcer will use to announce your entry as it passes the reviewing stand. If there are multiple entries, list them in the exact order they will appear in the parade. List names and titles of key personnel (i.e. commander, drum major, band director, vehicle driver, etc) and the size of the marching element. If entering a float, give description, theme, or name, length and height. Type of Entry choices include; float, auto, band, equipment, march unit, JROTC/ROTC. Use reverse side of page for additional entries.
