ENTRY FORM FOR 2007 NATIONAL VETERANS DAY PARADE Mail form to Parade Chairman, PO Box 642, Birmingham, AL 35201, no later than October 12. The Parade Order will be mailed the first of November. 1. Name of Organization: __________________________________________________ 2. Type of Organization: Veteran Military JROTC Band Civic 3. School Classification: Elementary Middle High College Military 4. Entry Information: This is the information the parade announcer will use to announce your entry as it passes the reviewing stand. If there are multiple entries, list them in the exact order they will appear in the parade. List names and titles of key personnel (i.e. commander, drum major, band director, vehicle driver, etc) and the size of the marching element. If entering a float, give description, theme, or name, length and height. Type of Entry choices include; float, auto, band military, equipment, march unit, JROTC/ROTC. Use reverse for additional entries.
